• Marked impairment in three or more areas of function, rendering the individual unable to perform useful activities; or extreme impairment in one or more areas due to a mental or behavioral disorder

When determining the compensation to be paid to an injured accident survivor, insurance companies assess all materials included in the claimant’s application. If the accident victim has sustained a catastrophic injury, a physician must complete the OCF-19 Form—an Application for Determination of Catastrophic Impairment.
